NSBMA will use the information submitted in this form to process registration or District Membership, communicate with district contacts and participating student board members, coordinate participation in NSBMA programming, provide access to materials and resources, support recognition opportunities, and maintain appropriate program records. Information may be reviewed by NSBMA staff, contractors, and facilitators involved in administering or supporting NSBMA programming. NSBMA will not sell student or district information and will retain registration and participation information only as long as reasonably necessary for program administration, record-keeping, follow-up, and evaluation.

I acknowledge that I am submitting this registration on behalf of my district/school system and, to the best of my knowledge, the district/school system has or will obtain any permissions, approvals, or consents required by district policy or applicable law for the listed student board member(s) to participate in NSBMA programming.
